Please read the instructions carefully to determine whether to submit form DS-11 or form DS-82.
DS-11: Application for a U.S. Passport
This form is to be used for:
- First-time applicants.
- Applicants replacing a lost or stolen passport must also complete Form DS-64.
- Applicants whose previous passport was issued before age 16 and applicants whose previous passport was issued more than 15 years ago.

You must submit the following with the form:
- Proof of U.S. Citizenship: for example, previous U.S. passport, original or certified copy of Birth Certificate, Report of Birth Abroad, original Naturalization or Citizenship Certificate.
- Photo Identification: for example, Photo ID card, previous U.S. passport, or driver's license.
- Family photographs should be submitted to establish the development of minor children; these photos should range from the age in which their previous passport was issued to their current age.
- Two (2) recent color photographs, 2"X2", on white background with full front view.
Note: See special requirements for children under the age of 16 below.
Fees (DS-11): non-refundable
- First time adult passport: US$ 135.
- Replacement for lost, stolen or damaged adult passport: US$ 135.
- First time child passport and renewal for applicants under age 16: US$ 105.
- First time adult passport for applicants whose previous passport was issued before age 16: US$ 135.
- Renewal of adult passport whose previous passport was issued more than 15 years ago: US$135.
- At the Embassy: Fees may be paid in U.S. dollars or the equivalent in Lempiras. Exact change is required. Credit cards are accepted. Personal checks are not accepted.
- At the Consular Agency in San Pedro Sula: Fees may only paid in Lempiras. Exact change is required. Credit and debit cards are NOT accepted. Personal checks are NOT accepted.
Special Requirements for Children under age 16
- Both parents (or the child's legal guardian(s)), as well as the child, must be present.
- Evidence of both parents' identities: passports, driver's license, or other photo identification.
- Child's original Birth Certificate.
- Child's current U.S. passport.
- Photographs should be submitted to establish the development of minor children; these photos should range from the age in which their previous passport was issued to their current age.
- If the applicant is a minor, born in the U.S., family photos of the child in the U.S. should be presented.
- If only one parent is present in Honduras, a notarized written authorization in English from the other parent is required. Form DS-3053 Statement of Consent from Absent Parent.
DS-82: Application for a U.S. Passport by Mail
Note: Requires personal appearance when issued in Honduras.
This form is to be used for:
- Renewal of 10 year validity passport.

You need to submit the following with the form:
- Most recent U.S. passport.
- Two recent color photographs, 2"x 2".
Fees: (DS-82): (non-refundable)
- US$ 110.
- Exact change is required, paid in U.S. dollars or the equivalent in Lempiras.
- International credit cards are acceptable at the Embassy only (not at the Consular Agency in San Pedro Sula).
- Personal checks are not acceptable.
DS-4085: Application for Additional Visa Pages
- Fill out Form DS-4085.
- Submit it along with your U.S. passport during normal American Citizens Services hours (listed below).
- There is a US$ 82 fee to have visa pages added to your valid U.S. passport.

DS-5504: Name Change, Error Correction, Replace Limited Validity Passport
This form is to be used for:
- Name change in a valid U.S. passport (less thanone year from date of issue). Note: If the name you are currently using differs from the name on your most recent passport, you must submit a marriage certificate, divorce or court order showing the change of name. The name change document must bear the official seal of the issuing authority.
- You need to extend the validity of a limited passport (if eligible).
- You need to correct a printing error in your valid U.S. passport (while passport is still valid).

General Information
Additional Documents: The Consular Officer may require documentation in addition to items listed above. In this case, the applicant has 90 days to provide the documentation. After the 90-day period, the application will be considered abandoned and will be sent to the U.S. Department of State.
Passport Photos: Submit two recent 2x2 inch, color photographs, taken within the last six months. The photographs must be in color, clear, with a full front view of your face, and printed on white background. The image size measured from the bottom of your chin to the top of your head (including hair) should not be less than 1 inch, and not more than 1-3/8 inches.
Social Security Number: You must provide your social security number on your passport application. Applicants who fail to provide a valid social security number are subject to a US$ 500 penalty enforced by the IRS. If you do not have a social security number, you should enter zeros in the box.
Public Hours
Tegucigalpa: If you apply at the Consular Section at the U.S. Embassy in Tegucigalpa, on Avenida La Paz:
- To schedule an appointment for a U.S. passport.
- Passports are issued at the National Passport Center in the U.S. and will be ready in approximately two weeks. You may pick up the passport at the embassy or receive it by Cargo Expreso.
San Pedro Sula: If you apply at the Consular Agency in San Pedro Sula (11th floor of Banco Atlantida "Oficina Principal" across from the Central Park):
- The Consular Agent in San Pedro Sula will accept passport applications Mondays, Wednesdays and Fridays from 12:00 p.m. to 4:00 p.m.
- All applications accepted in San Pedro Sula must be adjudicated at the U.S. Embassy in Tegucigalpa and, if approved, they will be sent to the National Passport Center for issuance. Passports will be returned, generally by Cargo Expreso, in approximately four weeks.
- The Consular Officer may require additional documentation or a personal interview at the Embassy.
