1. DS-11: Application for a U.S.
Passport
THIS FORM IS TO BE USED FOR:
• First-time applicants.
• Applicants whose previous passport was lost or stolen.
• Applicants whose previous passport was issued before age 16 and
applicants whose previous passport was issued more than 15 years ago.

YOU MUST
SUBMIT THE FOLLOWING WITH THE FORM:
• Proof of U.S. Citizenship: for example, previous U.S. passport,
original or certified copy of Birth Certificate, Report of Birth Abroad,
original Naturalization or Citizenship Certificate.
• Photo Identification: for example, Photo ID card, previous U.S.
passport, or driver’s license.
• Family photographs should be submitted to establish the development
of minor children; these photos should range from the age in which their
previous passport was issued to their current age.
• Three (3) recent color photographs, 2”X2”, on white
background with full front view.

for children under the age of 16 below.
FEES (DS-11): non-refundable
• First time passport: US$100.
• Replacement for lost, stolen or damaged passport: US$100.
• Renewal for applicants under age 16: US$85.
• Applicants whose previous passport was issued before age 16: US$100.
• Applicants whose previous passport was issued more than 15 years
ago: US$100.
• Fees may be paid in U.S. dollars or the equivalent in Lempiras.
• Exact change is required, no personal checks are accepted.
• International credit cards are acceptable at the Embassy only
(not at the Consular Agency in San Pedro Sula).
SPECIAL REQUIREMENTS FOR CHILDREN
UNDER AGE 16
• Both parents [or the child’s legal guardian(s)], as well
as the child, must be present.
• Evidence of both parents’ identities: passports, driver’s
license, or other photo identification
• Child’s original Birth Certificate.
• Child’s current U.S. passport.
• Photographs should be submitted to establish the development of
minor children; these photos should range from the age in which their
previous passport was issued to their current age.
• If the applicant is a minor, born in the U.S., family photos of
the child in the U.S. should be presented.
• If only one parent is present in Honduras, a notarized written
authorization in English from the other parent is required. Form DS-3053

2. DS-82: Application for a U.S.
Passport by Mail

3. DS-4085: Application for Additional
Pages
• Fill out form DS-4085.
• Submit it along with your U.S. passport during normal American
Citizens Services hours (listed below).
• There is no fee to have visa pages added to your valid U.S. passport.

DOCUMENTS
The Consular Officer may require documentation in addition to items listed
above. In this case, the applicant has 90 days to provide the documentation.
After the 90-day period, the application will be considered abandoned
and will be sent to the U.S. Department of State.
PASSPORT
PHOTOS
Submit three recent 2x2 inch, color photographs, taken within the last
six months. The photographs must be in color, clear, with a full front
view of your face, and printed on a white background. Instant and digital
photos are not acceptable. The image size measured from the bottom of
your chin to the top of your head (including hair) should not be less
than 1 inch and not more than 1-3/8 inches.
NAME CHANGE
If the name you are currently using differs from the name on your most
recent passport, you must submit a marriage certificate, divorce or court
order showing the change of name. The name change document must bear the
official seal of the issuing authority.
SOCIAL
SECURITY NUMBER
You must provide your social security number on your passport application.
Applicants who fail to provide a valid social security number are subject
to a $500 penalty enforced by the IRS. If you do not have a social security
number, you should enter zeros in the box.
PUBLIC HOURS
Tegucigalpa:
If you apply at the Consular Section
at the U.S. Embassy in Tegucigalpa, on Avenida La Paz:
• Passport applications may be submitted Monday through Friday from
8:00 a.m. to 11:30 a.m.
• Passports are issued at the National Passport Center in the U.S.
and will be ready in approximately two weeks. You may pick up the passport
at the embassy or receive it by Cargo Expreso.
San Pedro Sula:
If you apply at the Consular Agency
in San Pedro Sula (11th floor of Banco Atlantida “Oficina Principal”
across from the Central Park):
• The Consular Agent in San Pedro Sula will accept passport applications
Mondays, Wednesdays and Fridays from 1:00 p.m. to 4:00 p.m.
• All applications accepted in San Pedro Sula must be adjudicated
at the U.S. Embassy in Tegucigalpa and, if approved, they will be sent
to the National Passport Center for issuance. Passports will be returned,
generally by Cargo Expreso, in approximately four weeks.
• The Consular Officer may require additional documentation or a
personal interview at the Embassy.
