REPORT OF DEATH ABROAD OF A U.S. CITIZEN

There is no fee for this service. Original documentation is required. In order to obtain a Report of Death Abroad, the following documents must be submitted to the U.S. Embassy:

1. Medical certificate showing cause of death.
2. Death certificate from the Civil Registry (Registro Nacional de las Personas).
3. U.S. Passport.
4. Social Security Number.
5. Local address.
6. U.S. address.
7. Name of next of kin and address.
8. Please complete form on the reverse.

If the body is being transported to the United States or in transit to another country for burial, the following documents are required:

1. Medical certificate showing cause of death.
2. Death certificate from the Civil Registry (Registro Nacional de las Personas).
3. Passport.
4. Honduran Export Permit from the Ministry of Health.
5. Name, address and phone number of funeral home at place of destination.
6. Flight information. (Name of airline, date, port of entry, and name of carrier, flight number, departure and arrival times).
